Dog Paws Required Vaccinations & Forms

Age Appropriate Required Vaccines:
Please provide copies of all vaccines and negative fecal results PRIOR to boarding stay. You can email these copies to [email protected].
- Rabies (1 yr or 3 yrs)
- Distemper (1 yr or 3 yrs)
- Bordetella (annually)
- Negative fecal exam for each dog in house hold is required within 30 days prior to boarding stay then annually thereafter
- Proof of flea/tick prevention
Dog Paws Required Forms:
As part of your stay, all 4 forms below are required prior to any Boarding or Doggie Day Care 'PLAY'. These forms only need to be filled out one time per kiddo unless updates are needed.
- Boarding Intake Application: The boarding intake application provides us with the necessary information about your kiddo and their needs. It also informs of us your kiddo's veterinarian and medical history, as well as their emergency contact. This application helps us prepare to welcome your kiddo with open arms!
- Veterinarian Release and Liability Form: This form gives Dog Paws the authorization to take your kiddo to an external veterinary if necessary.
- Boarding Contract: This contract is an agreement between you and Dog Paws stating that you understand who we are, what we do, and the services we provide for your kiddo.
- End of Life Care Plan: This form states your requests for how Dog Paws should prepare for an unforeseen event with your kiddo.
